When you arrive at Prescott Preferred Chiropractic, a staff member will warmly greet you and present you with an intake form and medical history sheet that will help us better care for you. We will guide you through the forms, answer all your questions and help make sure the new chiropractic patient process runs smoothly.

We will take you on a short tour of the Prescott AZ Chiropractic clinic before you see Dr. Nelson. You’ll be introduced to the therapy area, learn about some of our programs designed to keep you well even AFTER your care, and help you become more familiar with who we are and how we can help you.

During your health history and examination, Dr. Nelson will discuss the issues you’re experiencing, any pain you’re in, and how you’re feeling that day. It’s important that you’re very thorough about what’s bothering you so he can recommend the correct care plan for you and get you back to optimal quality of life! Dr. Nelson may recommend digital x-rays or other tests that will help him decide the best course of treatment for you.
Your Report of Findings

After Dr. Nelson has studied your history, exam findings and x-rays he will thoroughly explain his findings, including and answer any and all questions you may have. His care plan will also be discussed at this time, and a team member will go over the cost and what will be covered by your insurance. You will be fully informed with no surprises.

Payment Options
Our care is very affordable. Many of our patients have no insurance....we never want finances to be the reason you are unable to accept care. Payment plans are available for extended treatment options. For the convenience of our patients, we accept the following payment methods:

Insurance
Insurance can be complicated. We are happy to verify your benefits as a courtesy to you. Please bring all necessary insurance information with you on your first visit. If there are benefits available we will inform you. Dr. Nelson is a participating provider for both Medicare and the VA, which means we bill Medicare or the VA and are paid by them directly. We choose to be out of network with all other insurance companies. This allows us to treat patients with what they need, rather than what their insurance will cover. We will provide superbills to patients that would like to submit claims to their insurance.
